I'm Andy Agadjanyan, PharmD — a California-licensed pharmacist (RPH 85803) with years of long-term care pharmacy experience and a member of the American Society of Consultant Pharmacists.

I started Universal LTC Pharmacist Consulting for one reason: most congregate living health facilities get their "consultant pharmacist" bundled free from their dispensing pharmacy — which means the consultant works for the pharmacy, not for you. Federal OIG compliance guidance for nursing facilities (November 2024) identifies free or below-cost bundled consultant arrangements as an anti-kickback risk area. And in practice, the documentation trail Title 22 requires often just doesn't get built.

I work differently. Your facility retains me directly, under a written agreement, and I answer to you. My job is simple: make your medication systems genuinely safe for your patients and make your documentation hold up when CDPH walks in.
